Medent Patient Portal
We now have a patient portal available for our patients use. This allows you to have real-time access to your medications, labs, and upcoming appointments. You will also be able to request appointments, request refills, and message your provider.
You will be sent a letter which will include an activation code. Once you receive the letter, find and click the Medent Patient Portal image on our website. This code is case sensitive and will need to be entered exactly as it appears. Make sure your caps lock is not on!
Once on the Medent Patient Portal site, click on the green video icon to view a tutorial on how to activate your account. Once you are ready, click on the link Activate patient portal account.
You will be promoted to fill out some basic personal information, as well as create a login name and password. Be sure to enter your name and date of birth exactly as it appears on your letter, including any suffixes. Please be sure you are entering your LAST NAME first. If any of this information is incorrect, you will be able to make changes once your portal account has been activated.
Please be as thorough as possible when filling out your personal information and medical history. This will save us time on the day of your appointment, and will help us provide you the best medical care possible.
